The University of Nebraska Medical Center ( UNMC ) is committed to visitors' right to privacy and confidentiality of personal information. UNMC will not collect personal information without the expressed consent of a visitor. This Privacy Notice ( Notice ) applies to personal information provided by users of mobile device applications developed or sponsored by UNMC. The Notice describes the types of personal information obtained about users, and how that information is used. We also describe the measures we take to protect the security of the information and how you can contact us about our privacy practices. This Notice applies solely to information collected by this App. Your use of this App indicates to us that you have read and accept our privacy practices, as outlined in this Notice.

This UNMC Application (the App ) is designed to assist a user in screening for the relative risk of COVID 19 disease caused by coronavirus. The App can also assist you if you wish to participate in a survey of symptoms by geographic region. This App is for a general audience in the United States, is not targeted to children, and we do not knowingly collect personal information from children under 13 years of age.

What data is being collected?

The App collects personal information by asking the user to share their name, date of birth, zip code, phone number ( demographic data ), medical symptoms, travel and potential exposure history and co-morbidity data. Data is initially collected and stored only on the user's device. During this initial collection phase, the user can opt-out of all demographic data collection by choosing the "prefer not to answer" option for each of these questions and still complete the screening. The App does not utilize user demographic data to make recommendations related to risk. One need not enter any demographic data to use the screener to its full capabilities. The App has a function which provides users with a Share Sheet of information that includes the responses to the screening questions, demographic data (if entered), and the recommendation generated from the logic tree. You may distribute this Share Sheet to third parties at your discretion.

The App will also separately ask you if you would like to participate in a survey of symptoms by geographic region. If you consent to participation in the survey you will be asked to provide your zip code, gender, date of birth, symptoms and any history of testing for coronavirus exposure or COVID-19 infection. The data submitted in connection with this survey will be de-identified; personal demographic data apart from zip code will not be collected or retained.

What purposes will the data serve?

The App will use data to assist a user in screening for the relative risk of COVID 19 disease caused by coronavirus. Risk recommendations are processed using a logic tree that has been created by clinicians and scientists at UNMC.

The App can also assist you if you wish to participate in a survey of symptoms by geographic region. If you elect to participate in the survey, UNMC will collect user de-identified regional symptom and testing data which may show patterns of disease progression.

Where is the data being sent?

If you do not elect to participate in the survey, UNMC will not store or analyze user data. Deleting the App will delete all data. Data is only stored in an app container. Personal information is retained for the period needed to fulfill the purposes for which the personal information was collected and as otherwise required or permitted by applicable law. If you do not elect to participate in the survey, UNMC will not be disclosing user data for any reason.

If you elect to participate in the survey, de-identified regional symptomology and testing data which may show patterns of disease progression, may be shared with third parties including but not limited to state and local health agencies.

The App has been designed with security measures in place to protect the loss, misuse, and alteration of the information. We maintain administrative, technical and physical safeguards designed to protect personal information against accidental, unlawful or unauthorized destruction, loss, alteration, access, disclosure or use. We make reasonbable efforts to ensure a level of security appropriate to the risk of the processing, taking into account the costs of implementation and nature of the processing of such personal information. However, no data transmission over the Internet can be guaranteed to be completely secure. Accordingly, we cannot ensure or warrant the security of any information that you transmit, including any Share Sheet produced by the App, so you do so at your own risk.
